About this journal

A new doctor slides a form across the desk. Current medicines? Allergies? Last blood pressure reading? Anything in the family? You know most of it, roughly. The rest is on an old prescription in a drawer and a lab result somewhere in your camera roll.

Health Chart is an adult health record journal that puts your medical history in one book you can actually find. It's a doctor visits journal for checkups, tests, sick days, and the questions you meant to ask, so you stop rebuilding your own history from memory every time you sit down with someone new.

Fill in the Basics over a few evenings with old prescriptions and whatever papers you can find. The snapshot page is the one you hand over, or read from, when a doctor asks. After that, the book fills in as life happens. Each of the 20 appointment records is a two-page spread, and the second page is for questions you write before the visit. Ten minutes goes fast, and the question you forget is usually the one you came for.

It's honest about what it is. It's a record book, not medical advice. There are no target numbers, no screening schedules, and nothing telling you what's normal. The blanks are for what your own doctor says, in their words, and anything urgent goes to them, not to the page.

What's inside

  • My Health Snapshot — conditions, medicines, allergies, and surgeries on one page for any new doctor
  • The Basics — PhilHealth and HMO notes, medicines copied from the label, medication changes, family history, and your care team
  • 20 appointment records — what was measured, what the doctor said, your questions, and their answers
  • Tests & Results — lab and imaging logs, vaccinations, and home readings if your doctor asked for them
  • 8 "When I'm Unwell" records — symptoms day by day, what you tried, and what the doctor said it was
  • Referrals, procedures, ER visits, and a costs and claims log — the paperwork that usually goes missing
  • The Question Bank and "What I Wish I'd Asked" — for the question you thought of in the jeepney home

It's for adults juggling more than one doctor, people managing an ongoing condition, and anyone tired of answering "I think so?" at the clinic. It's also a practical gift for a parent heading into their senior years, especially from the adult child who drives them to checkups and ends up answering the doctor's questions anyway.

Keep it going in the healthy years, too. A boring normal result from three years ago is exactly what the next doctor wants to compare against.
